The next Bitesize Learning session on the Transition to Adult Services will be held on Tuesday 24 February 1:15pm-2:00pm (UK times). Please CLICK HERE to register online. \nMoving from paediatric to adult healthcare is a critical stage for young people\, particularly for those managing long-term conditions and complex treatment plans. Medicines often play a central role in this journey\, and pharmacy teams are well placed to support safe\, effective\, and person-centred transitions. \nThis session will explore the key challenges young people face when transferring their care\, highlight best practice examples\, and share practical strategies that pharmacy teams can use to improve medicines optimisation at this interface. We will also discuss how services can embed young people’s voices into transition planning to promote continuity of care and a seamless transfer between paediatric and adult services. \nOur expert speakers: Ashifa Trivedi\, Sarah Mohamad \nBy the end of this session\, participants will be able to: \n\nExplain the importance of medicines-related support during the transition from paediatric to adult services.\n\n\nIdentify common challenges young people face with their medicines at the point of transfer\, including adherence\, unlicensed medicines\, and communication gaps.\n\n\nDiscuss practical approaches pharmacy teams can use to optimise medicines safety and person-centred care during transition.\n\n\nRecognise the value of involving young people’s voices in planning and delivering transition services to promote continuity of care and a seamless transfer.\n\nA gentle reminder that these online sessions are one of the benefits of NPPG membership.
