We have two excellent speakers – Venetia Simochowitz from GOSH and Uchu Meade from St Marks Hospital who have both recently completed their consultant pharmacist accreditation in nutrition. \nThis 1-hour introductory session will support aspiring consultant pharmacists to begin developing a consultant portfolio. It will outline the purpose and structure of a consultant pharmacist portfolio\, explore how nutrition practice aligns with advanced competency domains\, and highlight suitable evidence such as reflections\, case studies\, audits\, and service development work. There will be opportunity for discussion and questions. \nBy the end of the session\, you should leave with practical next steps to start or strengthen your consultant pharmacist portfolio. Medicines often play a central role in this journey\, and pharmacy teams are well placed to support safe\, effective\, and person-centred transitions. \nThis session will explore the key challenges young people face when transferring their care\, highlight best practice examples\, and share practical strategies that pharmacy teams can use to improve medicines optimisation at this interface.
