Moving from paediatric to adult healthcare is a critical stage for young people, particularly for those managing long-term conditions and complex treatment plans. Medicines often play a central role in this journey, and pharmacy teams are well placed to support safe, effective, and person-centred transitions.
This session will explore the key challenges young people face when transferring their care, highlight best practice examples, and share practical strategies that pharmacy teams can use to improve medicines optimisation at this interface. We will also discuss how services can embed young people’s voices into transition planning to promote continuity of care and a seamless transfer between paediatric and adult services.
